Who qualifies for this plan?
UHC Care Advantage could be a fit for you or your loved one if the following criteria are met:
- You are a Medicare beneficiary entitled to Medicare Part A and enrolled in Part B
- The level of care is met as determined by the state (this is an evaluation of medical, cognitive and functional needs and abilities)
