UHC Care Advantage, an IE-SNP from UnitedHealthcare, offers prescription drug coverage, vision, dental, transportation benefits and more. The plan works with you, your family and your provider to deliver customized care with guidance from a dedicated team. Here are some highlights:

Dedicated care team

The plan features a dedicated care team to customize care for your needs.

Vision coverage

Benefits include allowance for eyewear, plus routine eye exam and lenses.

Over-the-counter (OTC) products

Members receive credit to purchase OTC products.

Hearing coverage

The plan includes an annual routine hearing exam plus hearing aids credit.

Routine podiatry

Benefits include foot care visits and other covered preventive care.

Dental coverage

Allowance for covered services like routine cleanings, x-rays and more.

Transportation assistance

Members receive transportation to or from doctor visits and the pharmacy.

Who qualifies for this plan?

UHC Care Advantage could be a fit for you or your loved one if the following criteria are met:

  1. You are a Medicare beneficiary entitled to Medicare Part A and enrolled in Part B
  2. The level of care is met as determined by the state (this is an evaluation of medical, cognitive and functional needs and abilities)
  3. Reside in one of the plan service areas
